Job Summary

DUTIES OF THE ROLE SHALL INCLUDE:
The duties may include but are not limited to the following and the post holder may be required to perform other duties as appropriate to the post which may be assigned to him/her from time to time and to contribute to the development of the post while in office.
The role is based within the Housing Department of Kilkenny County Council, mainly within the Homeless Services Unit.
The successful applicant will fulfil the following duties;
5.1.1 Carry out assessments of clients that present to the Council stating that they are homeless.
5.1.2 Prioritise homeless prevention at assessment stage and carry out mediation and intervention measures where necessary.
5.1.3 Arrange for temporary emergency accommodation within the existing services available should it be required.
5.1.4 Case manage families and individuals in emergency accommodation including B&B/hotel accommodation, or those in certain other types of emergency accommodation that require support. Coordination and management of fortnightly clinics, held in office or on site, in order to maintain engagement, assess needs and manage exit strategies of families/individuals in various emergency accommodations.
5.1.5 As Case Manager be responsible for engaging the clients who are homeless to identify their individual support needs, help them engage with appropriate services and develop and implement an exit strategy from homelessness in each case.
5.1.6 Work closely with the HAP Place Finder Service on homeless prevention and achieving exit goals for homeless clients in emergency accommodation.
5.1.7 Refer the clients to appropriate community or health services.
5.1.8 Work as part of the Homeless Action Team (HAT) to meet the needs of the homeless clients, or those at risk of homelessness.
5.1.9 Work in consultation with other support services and specialists to meet the needs of the homeless clients.
5.1.10 Maintain a caseload of clients and offer clients ongoing support.
5.1.11 Be responsible for keeping up to date casework records and written assessments, and ensure all necessary information is recorded on the Pathway Accommodation & Support System (PASS), ihouse and other systems as required.
5.1.12 Provide progress reports as required in relation to caseload.

5.1.13 As part of the Homeless Action Team (HAT) attend weekly meetings and provide information as required
5.1.14 Attend inter-agency meetings and reviews of clients as and when required.
5.1.15 Participate in relevant training and development courses as agreed with your supervisor.
5.1.16 Undertake other duties as might be reasonably assigned from time to time in consultation with your supervisor.
